GENERAL SECURITY INSTRUCTIONS
In order for Bartlett to request unescorted access on your behalf we must perform a background investigation. It is your responsibility to:
- Choose and print the correct security personal history questionnaire (PHQ) from the selection below.
- Read the instructions located on the PHQ and complete in black ink as required.
- Ensure the information is legible, accurate and complete.
- Review your information to assure there are no gaps in the required time frame and all required information has been provided.
- Sign, date and fax completed PHQ to the security department no more than three (3) weeks prior to your start date.
SECURITY FORMS
Note: All Bartlett forms are in pdf format. If you do not have Adobe Reader, click on the icon in the top right hand corner for a free copy.
- Initial PHQ - If you have never been granted unescorted access OR your last access was more than 3 years ago OR your last access terminated unfavorably.
- Update PHQ - If your last access was terminated favorably more than one year ago, but less than 3 years ago.
- Reinstate PHQ 365 - If your last access was terminated favorably within the past 365 days.
- Reinstate PHQ 30 - If your last access was terminated favorably within the past 30 days OR if you currently hold access AND have been requested by security to complete a PHQ.
- Bartlett Consent - If you need the most recent revision of just the Bartlett Consent form.
