Security PSEG

In order to gain unescorted access your background must be completed.

IF IT WILL BE GREATER THAN 25 DAYS BETWEEN WHEN YOU WERE LAST BADGED AT A NUCLEAR PLANT AND YOUR START DATE AT SALEM/HOPE CREEK, it is your responsibility to:

• Review your information to assure there are no gaps in the required time frame and all required information has been provided. Using the quality checklist will help insure accuracy and completeness.
• Ensure the information is legible, accurate and complete.  Any questions you may have can be answered by one of the PSEG security personnel at the phone numbers listed on the cover sheet. Sign date and return your paperwork, including the cover sheet, as noted below.

COMPLETED SECURITY FORMS

• Fax all completed security forms to PSEG SECURITY BADGING @ (856)339-5620.You may also choose to mail the completed forms to the address below, but all completed forms must be received by PSEG 14 days (2 weeks) prior to your start date.

SECURITY FORMS

• Initial PHQ - If you have never been granted unescorted access OR your last access was more than 3 years ago OR your last access terminated unfavorably. 
• Update PHQ - If your last access was terminated favorably more than one year ago, but less than 3 years ago.
• Reinstate PHQ 30 - If your last access was terminated favorably within the past 30 days OR if you currently hold access AND have been requested by security to complete a PHQ.
• Reinstate PHQ 365 - If your last access was terminated favorably within the past 365 days.
